Forest Hill Admissions

Welcome! My name is Peggy Ouellette, LSW. I am the Social Worker at Forest Hill. When you, or a family member, are admitted to our facility, I will be the one of the first to greet you. I handle all admissions and discharges. Some of my other duties include helping to fill out any necessary paperwork, visiting residents on a regular basis, and reviewing eligibility requirements.

PeggyI also provide support to residents and their families who are coping with chronic, acute, or terminal illnesses. Together we work out any problems that may stand in the way of the patient's recovery or rehabilitation. I help plan the patient's needs after discharge by arranging for home services such as meals-on-wheels, home health services, oxygen supplies, and rehab equipment.

Our facility offers a support group for families of Alzheimer's disease. The meetings are held on the 2nd Tuesday of every month from 6:00-7:30pm in the downstairs level of Forest Hill.
