Forest Hill’s mission is to produce the best outcome possible for our patients. Serving the greater Fort Kent area since 1974, Forest Hill Rehabilitation and Skilled Nursing Center became a department of Northern Maine Medical Center in 1991. Licensed for 45 beds, Forest Hill offers long term care and skilled rehabilitation in a home-like atmosphere. Forest Hill Rehab and Skilled Nursing Center provides rehabilitative, restorative, and ongoing skilled nursing/long term care to patients or residents in need of assistance with activities of daily living.

An individually tailored program of care is developed for each patient by our team of highly qualified individuals. By promoting an environment of respect, dignity and confidentiality, our goal is to exceed the expectations of the people we serve.

In 2019, 2020,2021 and 2022 Forest Hill achieved overall five-star recognition from the Centers for Medicare and Medicaid Services.

Forest Hill Rehabilitation and Skilled Nursing Center has also been recognized as a 2019 recipient of the Bronze – Commitment to Quality Award by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL) for its commitment to improving the lives of residents through quality care. The distinction is the first of three progressive award levels through the AHCA/NCAL National Quality Award Program. The program, presented by the leading association in long term and post-acute care, honors association members across the country that have demonstrated their dedication to improving quality of care for seniors and persons with disabilities.

Since our services are based on need, not choice, Forest Hill’s goal is to make rehabilitation as pleasant and rewarding as possible for each patient. One way we do this is by accepting admissions seven days a week.  Another way is through our skilled rehabilitative services, where our nurses are here around the clock to provide pain management, specialized wound care and intravenous (IV) therapy.  Our physical, occupational and speech therapists are on site six days per week. It is a proven fact that patients improve with skilled therapy intervention. Our goal is to restore patients’ functions and return them to the home environment.

Occasionally a patient will need to stay at Forest Hill longer than anticipated.  We can accommodate, based on availability, short and long-term placement. There are many activities for our patients planned each month.  Our facility is also equipped with a computer and Internet access for our residents to enjoy at their leisure.  We strive to make our facility feel like home as much as possible.

Alzheimer’s Walk

The Alzheimer’s Association’s Caregiver Support Group is designed to provide emotional, educational and social support for caregivers through regularly scheduled meetings. The support group encourages caregivers to maintain their own personal, physical and emotional health, as well as optimally care for the person with dementia.  Since the start of the COVID-19 pandemic, support groups have taken on a virtual format for your safety.

Each group is posted online at alz.org/crf and registration is required.  All of the upcoming groups are sent each month with the link to register embedded in each one.
