Care Coordinators

Northern Maine Medical Center’s RN Care Coordination program provides necessary assistance to patients by promoting preventive care to improve well-being and self-management of chronic, complex disease processes.

RN Care Coordinators reach out to patients who have had an ER (Emergency Room) visit or have recently been hospitalized. RN Care Coordinators support patients in the decision-making in their health management.  

Northern Maine Medical Center’s Care Coordinators provide patient education on disease processes, medications, treatment plans and assess patient needs for follow-up care or available community benefits. The objective is for the patient to receive the best quality of care and optimize wellbeing. 

Diabetes Education

Northern Maine Medical Center’s Diabetes Education Program is a comprehensive outpatient diabetes education program in compliance with the Maine CDC Diabetes Prevention and Control Program accredited by the Association of Diabetes Care & Education Specialists.  Our goal is to provide quality, comprehensive diabetes self – management education and support necessary to empower the person with diabetes to better manage his or her disease. 

Our diabetes program is referral based. Our team consists of RN Diabetes Care & Education Specialists and registered Dietician. Combination of outpatient group and individual educational sessions along with medical nutrition therapy prove to help improve overall health and successful management of diabetes. Managing diabetes includes the following 7 self-care behaviors: healthy eating, being active, monitoring, taking medications, problem solving, reducing risks and healthy coping. We see patients from all age groups with Type 1, Type 2, Pre–Diabetes and Gestational Diabetes.

Other services offered within our DSMES program are:

  • Insulin pump therapy and management
  • Continuous Glucose Monitoring Systems
  • Meal Planning and Carb Counting counseling
  • Gestational Diabetes counseling

We offer DSMES program at both our Fort Kent and Madawaska locations.
